What Causes Itchy Spot On The Toe With White Head?

I have a spot on my big toe that looks like a bout with a white head and it itches very bad..my toe has become red and there are 2 smaller spots starting. It woke me up in the night two nights ago. I've put anti itch cram on it and have taken Benadryl.
