What Causes Lack Of Concentration?

Hi sir, I m Madhav. the main problem from which m suffering from so many years is that I cant concentrate my mind. My mind always think in four or more sectors. Even I cant sleep sound. Complete duration while m sleeping even night and day the mind being on thinking something something but never take a rest . Help me sir plz My mob no -0000 My email id - YYYY@YYYY
