What Causes Lump In Throat Along With Tightness In Neck And Shoulder?

I get a lump in my throat and my neck and shoulders get tight my head and face hurt and my ears bother me im dizzy and cant think straight at all. My ears feel like there full of peanut butter ive been to the Ent. Please help oh and ive had problems with acid reflux please help
