What Causes Lump On Tongue With Radiating Pain?

I have had a bump on my tongue for the last 15 years and it would always swell and become enlarged when I ate anything with tomatoes in it. It had recently started to come loose and I pulled on it and several "pieces" came off it. Ever since, I have had pain on my tongue and it radiates down into my neck and makes my tonsil area hurt. What type of physician would treat this? Could this be something serious?
