What Causes Mildly Red, Itchy And Scaly Skin Rash?

I found out this summer I have stage III renal disease, last year ANA of 1200, RA negative, renal biopsy just shows tubule damage, creatinine level ranges from 1.04 to 1.27 at any given time. I have swelling of fingers in hands. My physician does not that I have Raynaud's, in bath water my feet have black spots. Over the past few days I have an extremely dry mouth. I feel that I have gone to everyone, rheumatologist, nephrologist next is the dermatologist about this rash on my face. It is mildly red, itches and scaly and metrogel does not make better.
