What Causes Mucus Plug Along With Blood In Bowels?

Hi my wife TTP patient followed by DVT and currently under medication of Warfarin 5.5 mg. Last two days she noticed mucus with blood stain before passing stool on two times in a day. ER doctor screened CBC and INR and all are normal. Please advise what is the reason.
