What Causes Muscle Spasm Above Right Eye Even After Botox Treatment?

I am a 51 year old female. I developed a muscle spasm above my right eye (in the forehead) about a year ago. It stays contracted and does not go down. It has come back 3 months after being treated with botox. Any ideas on causes? CAn a very deep dental cavity and infection on that side of face be related?
