What Causes Needle Like Hole On Penis?

Last week I had a small inflamtion on the piece of skin at the back of the foreskin that attaches this to the penis when you pull this back. over the weekend this then appeared with a small needle like hole, it was raw and sore, no discharges etc, it has no healed over in 2 days but remains sore, slightly inflammed and bigger then normal - not in a uncomfortable way though. What is this and what should I do. Im 6ft normal weight/health good.
