What Causes Nose Bleeding Along With Severe Cough?

Hello, I'm 16 and I'm quite worried about my mother to a point where I'm constantly thinking about it. She's in her late 30's and has been smoking from the age of 12. Basically, I walked into the bathroom and she was having a nose bleed, it wasn't a heavy nose bleed more of a steady one but I think they have been happening often. If she begins to cough she carries on for at least five minutes and they are quite harsh and loud, it sounds like her throat is ripping apart when she coughs. She has been throwing up a lot too, sometimes she'll be coughing for a while then I'll hear her throw up, the problem with this symptom is that I don't know if she's forcing herself to throw up or if it's involuntary and is caused by the coughing. She often complains about being shakey too, is there something serious going on here and should I tell her to visit the hospital, or is it just a strange cold.
