What Causes Nose Bleeds And Headache?

I have been having headaches everyday for the past maybe 6 months. On top of that is dizziness within the beginning of all of that (but much more used to it and not so dizzy anymore), memory loss, and now I just got my 3rd nose bleed within less than a week. Can you please tell me what's going on?
