What Causes Numbness In Upper Shoulder And Chest?

I have been having severe numbness in my upper right shoulder and chest area, especially around my neck and ear area almost like it had fallen asleep and then was getting the pin like pricking feeling. It has been going on for quite some time, over 6 months, and now seems to be getting worse. My vision and thought process seem to be getting slower/poorer.
