What Causes One Sided Chronic Headache?

I have suffered with chronic headaches for 5 years. The have intensified to daily events. I have been treated with epileptic meds little relief. The pain is concentrated to one side on my head and face. It is sever enough to cause speech and thought interruptions I recently had an MRI showing multiple scattered, predominantly peripheral foci of abnormal signal intensity within the Centrum semiovale and corona radiate bilaterally, overwhelming likely ischemic. I need to know what is causing this and how to make it stop. Ideas?
