What Causes Pain And Swollen Glands Of Neck?

I have been bothered for about 3 years now with occassional pain and swelling in the glands of my neck. This is accompanied with pain in the inner ear, jaw, head and neck. I also get a feeling like someone has their thumb on my throat making it difficult to swallow and causing changes in my voice. I have been to an ENT and he found nothing. Any ideas where I should go next?
