What Causes Pain And White Dots On The Nipples?

My nipples are very very painful and i noticed that right in the middle (the bit that sometimes sticks out more) has pale yellow-white dots on it, they aren't raised and i don't think they contain any puss but i don't want to squeeze it as it is far to painful. I don't know what it is or even if the pain is linked to the dots at all, wanted your opinion as i don't get on with my doctor.
