What Causes Pain In Breast Along With Itchy Rashes Under Nipples?

I'm a 31 yr old female since having a miscarraige 2 yrs ago my periods are very irregular every 3/4 months . Each month I get all the symtoms of menus tenderness in breasts, fluid retention, emotional and cramping .. But don't get any period ... I'm not on any contraceptive or medications I've had a pelvic ultrasound to rule out pocs and endometriosis .. My main problem at the min is my breasts are more sensitive than normal / painful to touch and dry itchy patches underneath the nipple area also one nipple is out of shape compared to other one .. I have checked for lumps but none present
