What Causes Pain In Jaw And Neck Radiating To Chest And Left Arm?

In the lastseveral days I have had jaw and neck pain also reaching to chest and left arm . I have CHF , a 5 way by pass 13 years ago and a stint may 2013. Im thinking maybe its from bad tooth but no tooth is particularjy sore, Have also experienced fatigue and genereal weakness in the last seveal days.
