What Causes Pain In Left Shoulder Radiating To Elbow Along With Dizzy Spells?

I am experiencing left arm pain in my shoulder and it radiates down to my elbow. An when I sit or stand gets almost intollable. Laying down it quits. And been having dizzy spells. Sense yesterday. And I have had a heart attack in the past. But didn t have these symptoms. I m concerned
