What Causes Pain In Neck And Shoulders With Numbness In Arms?

I have pain in my neck, right shoulder, bend in my left arm, bend in my legs, pain in my right big toe. It is constant and severe. I have numbness in both arms especially when I lay down. MY right arm feels like someone punched me and the pain travels.
