What Causes Pain In Scrotum While Having Hydrocele On Testicle?

i have had a "hydrocele" on my testicle for about 15 years. Its like a dumbell shaped testicle. I went pee and felt like i got kicked in the nuts and my one testicle that was split into two ( the hydrocele testicle) feels flat like it popped. I am in about 2/10 pain in my scrotum. Is this a medical emergency or should i present to the e.r. immediately
