What Causes Pain In Testicles And Penis?

Hi, my boyfriend says his penis and his testicles hurt because he built up to an orgasm and then stopped himself from coming too many times. He says it s like blue balls but more severe, and has the slight feeling of needing to both urinate and excrete. What is this/what do we do, he says pain is moderate and ejaculation won t help because he can t ejaculate at this point
