What Causes Pain In The Penis During Intercourse?

hello...i am having a pain when i try to make sex with my wife without any protection..
the top part of my penis is in pain when it's ejaculated and it doesnt erect completely...
plus after that i feel pain when i want to urine...
could u please help and advice?
