What Causes Pain On Left Hand And Back?

Hello,so i ussualy have pain on my left hand and back also left side of my neck and the vein hurts.My bac losses balance while i sleep and my heart beats slow or fast n itmakes sleeping difficult everyday.Also i feel my chest and shoulders numb n cold how do i know if it panic attack n not some serious stuff
