What Causes Painful Blister On The Shoulder?

I have a dime size pimple on my back right by my shoulder blade, at 1st I thought it was a scratch due to my bra strap. I am now wearing a sport bra in order to not cause irritation. But the pimple sometimes wakes me up because it hurts ? What can the cause be ? and should I see a Dermatoligist?
