What Causes Painful Body Jerks?

I have these body jerks that are so hard out hurts. I may had surgery on my ankle I fractured it but every time I get this jerk it s like a muscle spasm on my foot or ankle and it hurts so bad. what could cause these body jerks. I also get them really bad as I m laying down to sleep. is this normal?
