What Causes Painful Breast?

My mom has severe pain in her left breast. She says in the nipple. There is nothing visible, no rash, no redness or discoloration, no lumps, just soft tissue that hangs down. She is 88. We give her Tylenol and it doesn t help. We limit Advil because she has a kidney issue. Any idea what it is or how to treat pain?
