What Causes Painful Cracks On Penis Foreskin?

There are cracks on the foreskin of my penisvand they are very painful. The foreskin does not pullback. I am diabetic and elderly. Should I seeknsurgical help to circumcise myself. Is it very painful.My age iS seventy five and weight is one hundred and ten kilos and heisht is six feet.
