What Causes Painful Finger?

33 month old boy came home from day care with clenched fist, saying his finger hurts. His appetite is normal, voiding is normal, laughing and enjoying himself. He did have a hive like rash yesterday, the doctor looked at it and said nothing was wrong. The fist is something new. Should we be worried.
