What Causes Painful Headaches Behind Left Eye?

i get extremely painful headaches behind the left eye, it feels like somebody is trying to push my eye out from the inside with a hot poker. they often hit at the same time of day, for several weeks, then go away for up to a month at a time. its more painful than being peppersprayed in the face with the eyes open. they rarely last more than an hour at a time.
