What Causes Painful Intermenstrual Bleeding?

hi im a 39 year old women and i started to suffer very bad periods, which will last up to 10 days. but i also suffer bleeding between periods and get very bad pain when i go to the toilet. i have noticed a swelling on my left side of my tummy, i am getting so down with this pain because it is so bad.
