What Causes Painful Knot At The Site Of Dog Bite?

I was bitten by a dog 2 weeks ago on the hand,wrist and forearm, and I now have a hard painful knot under the skin by one of the puncture wounds. It also sends pain shooting up my arm and its very painful to touch. Is this something I should worry about and what could it be.
