What Causes Painful Rash Under Arm?

hello, Last summer i got a rash under my arm and was real painful. it went away tho. now it has returned and is worse. it covers a big circuler area under arm that is pink and painful. it also has a white crust around it. I am 25, 6 ft tall 400 lbs and have severe autoimmune problems.
