What Causes Painful Swelling In Legs With Red Patchy Marks On Skin?

I am a heart patient & also have had L-4 to s-1 fused . I now have so much swelling in my legs so bad you can't see my ankles & swelling in almost whole body hand's get very bad I can't stand the pain anymore . I also get red patchy mark's on my skin my right leg is real bad & it & my hand's stay freezing cold about 75% of the time. Do you know why this is happening .
