What Causes Penis Glans Spots, Neck Tightness, Jaw Choking Feelings After Intercourse?

dear doctor, I had had protected sex with a sex worker on march 12th. the condom tore off during the act and i noticed it after some time only. i did not come at all inside her. now on 19th april i noticed some red spots around the head of my penis. also i'm having a feeling as if my neck is getting tightened and the portion below my jaws are feeling like chocking my neck. i smoke 6 cigarettes a day and i was having neck pain even before i had done the sex act. i didn't get any fever or illness though. is it likely that i might have been infected? should i get a blood test done? this is a picture of my penis WWW.WWWW.WW
