What Causes Persistent Cough Along With Vomiting?

I am talking to a girl in the Philippines who's nephew has had a bad cough for 4 days. She says he doesn't like to eat and when he coughs he vomits. I asked if he had a fever and she said he was hot, which probably means he does. She doesn't have a thermometer. I don't know if the vomiting has been occuring for all the time he's had the cough.
Does this mean he'll have to go to the doctors over there and or a hospital. He is 3
