What Causes Persistent Headaches And Throbbing In The Eye?

my boyfriend has been having continuous headaches/migranes and throbbing of his left eye. he just finished surgery a month or so ago on his knee. he works out twice a day for about three hours. including playing basketball. he goes early in the morning then late at night. what could be the cause of this...sinuses? stress? another problem?
