What Causes Persistent Sore Throat Along With Fever And Chills?

I have been having a sore throat on and off for the last 2 months. I have taken z-pack. It goes away for 3-4 days and then comes back. With the sore throat I get fever and chills and I feel like my head is always under water. Aches at the base of my neck, at the jaw line and headache. Help.
