What Causes Pin Size Hole On Shoulder?

My wife has a small pin size hole in her skin on her shoulder. It appears black and has twice in the past had a black substance somewhat like sebum come out of it when she squeezes it. It has not gotten any bigger and has been on her shoulder for a couple of years now. just curious what it could be.
