What Causes Pressure Radiating From Shoulder To Neck Area?

When bending waist, & head going in & out of a sand trap while playing golf, I get pressure rises from my shoulder up my neck & head become lightheaded and on 2 occasions fainted couple of seconds but revived quickly & stood up. What may be the physical cause?
