What Causes Prolonged Periods Along With Swollen Breasts Post Bilateral Salpingectomy?

Hi , I had a bilateral salpingectomy 7 months ago and my periods were fine for the first 3 month s but since about November/October time they have been erratic I am currently on my monthly cycle and have been for a month now but it s only spotting except when I pass water and its thick heavy clotting . my breast are swollen as is my stomach I have gained weight which is unlike me as I excercise and eat a healthy vegetarian diet do you think this is a cause for concern.
