What Causes Pulsing And Tightening Of Muscles Around The Vocal Cords?

I am 58 years old, have been having the feeling of my heart beating in my throat and sometime it feels like it skips a beat, I have acid reflux and a voice disorder where the muscles tighten around my vocal cords and it makes me tired to talk, I am wondering if I could need some hormones for the heart problem?
