What Causes Pus Collection In Third Layer After I&D?

i jz had I&D for perinal abscess end of last january....nw im still feel the pain and after consult to the colorectral surgeon, they said there pus collection again in third layer. they recommend for another I&D...so Dr. explain why this happen?
