What Causes Radiating Pain From Sternum To Right Shoulder Blade?

i have terrible chest pains, starting at the sternum and pushing through to the shoulder blades, usually the right shoulder. when this happens, t feel hard of breathing,as is i am about to choke!! pains sometimes stay in the middle, between the shoulder blades.
