What Causes Raised Red Sore On Thigh And How To Treat It?

My husband has a quarter sized, slightly raised, somewhat crusty red sore on the back of his thigh. He has had Pyoderma Gangrenosum in the past, this looks very different. He also suffers from chronic Pancreatitis. What could cause such a sore to appear and does it need medical attention immediately?
