What Causes Rash On Ankle?

I left Korea in 1968 with a 1/2 dollar sized rash on my ankle,last year I used Triamcinolon which took it away,2 weeks later I easily pulled a dime size black thickness from the arch of the same foot,now I have the rash back in the same spot where I pulled the black spot off what can this be?
