What Causes Rash On Hand Post Injury Despite Having Metal Implant?

My husband fell 4 months ago and broke his elbow infact he did not have an elbow after the fall onto a cement garage floor. The question is the doctor did a metal inplant and he now has a rash ...which nothing seems to work -- as an outward application. What do you think could help?
