What Causes Rashes And Itchiness On Toes?

My ring toe on my rt foot started itching and had a red rash about a month ago and now it has spread to all of my toes and plus there are red bump and the rash and two of my toes are blue. I went to the doctor and he said my foot was warm, pulses were good, and blood return was two seconds when he pressed on my blue toe. The .1% hydrocortisone cream isn't working.
