What Causes Rashes On The Forehead And Neck Of A 4 Year Old?

My 4 y.o. son has a raised rash over his trunk, neck, forehead x one week. None on his arms or legs. He seems fine and no fever nor cold symptoms, but getting worse. No one else in the family has the rash. He had his immunizations. Should I be concerned?
