What Causes Rattling And Shortness Of Breath In A Diabetic?

my wife just got out of the hospital on Saturday, low blood sugar, low iron, low potassium, high blood pressure, it what they said she had, she also has balance issues and trouble with her eyesight. she has type II Diabetes. now she has a rattling breath and can't breathe when she lays down, should I take her back to the hospital
