What Causes Re-occurrence Of Blood Discharge In Stool And Urine?

I have had a history of bleeding from the bowel for the last 9 months. Had a colonoscopy and it showed some serrated polyps which they removed and required to have tri annual colonoscopies. I am still occasionally bleeding from the bowel sometimes squirting other times just small amounts. I am starting to notice when passing urine and stools occasionally there is pink to red fleshy pieces present in both urine and stool. I am getting occasional abdominal cramping and back pain. Should I be concerned?
