What Causes Red Itchy Flaky Spot On Scrotum?

for years I ve had a red, itchy and (at times) dry and flaky patch on the left side of my scrotum. never spreads, just a localized 1.5 in diameter patch which drives me crazy with how much it can itch. have tried a few antifungal creams that target athlete s foot/jock itch (last one was clotrimazole) but none seem to help. thought on how to treat this? thank you!
